Parent Release Policy

Makeup Consent
AngelleSkai Kreations, LLC, requires you the Parent to print and sign this waiver. As part of the Spa
experience, age-appropriate makeup and spa treatment products will be applied. This includes but is not
limited to lip gloss and eye shadow. Makeup products used are hypoallergenic and makeup applicators
are new and disposable. I acknowledge that any of the beauty or “spa treatments” are for
entertainment purposes only and AngelleSkai Kreations, LLC are not licensed cosmetology professionals.
AngelleSkai Kreations, LLC will not cut, file, trim, push back cuticles or do any services provided at a
certified nail salon. AngelleSkai Kreations, LLC is an event planning company, and the spa party themes are
for entertainment only.

Photo Release Form for Minors
I giveAngelleSkai Kreations, LLC, permission to use my child’s likeness, image, voice, and/ or appearances
as such that may be embodied in any pictures, photos, video recordings, digital images, and the like,
taken or made on behalf of AngelleSkai Kreations, LLC, party activities. I agree that AngelleSkai Kreations,
LLC has complete ownership of such pictures, etc., including the entire copyright, and may use them for
any purpose consistent with AngelleSkai Kreations, LLC Parties. These uses include, but are not
limited to, reprints, publications, advertisements, social media and any promotional materials in any
medium now known or later developed, including the internet. I acknowledge that I will not receive any
compensation, etc for the use of such pictures, etc., and hereby release AngelleSkai Kreations, LLC, from
any and all claims which are in any way connected with such use. I have read and understood this
consent and release.

Food Allergy Waiver
Your child will be participating in an entertaining baking or spa-like activities. Some of these activities
involve food products as part of the experience. We know that many children have allergies and some of
these allergies can be life threatening. We take many precautions to ensure that all the children are
safe. AngelleSkai Kreations, LLC will not be held responsible for any allergic reaction or life-threatening
incident including death that may occur. If your child has any known food allergies or intolerances that
will prevent him/her from participating in these activities, we will provide an alternative option at your
request.

Coronavirus/COVID-19
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other
public health authorities still recommend practicing social distancing.
I further acknowledge that AngelleSkai Kreations, LLC has put in place preventative measures to reduce
the spread of the Coronavirus/COVID-19.
I further acknowledge that AngelleSkai Kreations, LLC cannot guarantee that I will not become infected
with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by
the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others,
including, but not limited to, Party staff, and other guest and their families.
I voluntarily seek services provided by AngelleSkai Kreations, LLC and acknowledge that I am increasing my
risk to exposure to the Coronavirus/COVID-19. I acknowledge that I and my family must comply with all
set procedures to reduce the spread while attending a AngelleSkai Kreations, LLC event.

LIABILITY
We Strictly provide entertainment. Please be aware that children need to be supervised. We cannot be
held responsible for accidental damage caused by your child. AngelleSkai Kreations, LLC, assumes no
responsibility or liability for accidents or damage caused during the party. In the event of damage or
loss or our inventory and/or damage or loss to the party trailer, the responsibility lies entirely with the
client and or guardian. AngelleSkai Kreations, LLC, is not responsible for allergic reactions and / or any
medical injuries from the food, beverages, wearing costumes or any activity taking place before, during
or after the party.

I hereby release and agree to hold AngelleSkai Kreations, LLC harmless from, and waive on behalf of
myself, my heirs, and any personal representatives any and all causes of action, claims, demands,
damages, costs, expenses and compensation for damage or loss to myself and/or property that may be
caused by any act, or that may otherwise arise in any way in connection with any services received from
AngelleSkai Kreations, LLC. I understand that this release discharges AngelleSkai Kreations, LLC from any
liability or claim that I, my family, or any personal representatives may have against the business with
respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from,
or in connection to, any services received from AngelleSkai Kreations, LLC. This liability waiver and release
extends to the AngelleSkai Kreations, LLC together with all owners, partners, and employees.
